Consultations It greatly helps the consultation process if you could fax ahead of time the Veterinarian Referral Information Sheet (even if a referral will not be made) and indicate when you will be calling our office. This helps in quickly and efficiently obtaining the information our staff and ophthalmologist needs, to give you the most timely and best-informed response for your patient. Please also indicate any additional phone numbers we need in order to reach you, such as your back line and/or cell phone numbers.
