In order to provide the highest quality service to you, your patient, and your client, we request the following guidelines be followed when referring a case to Animal Eye Care:

• We provide Patient Referral Information Sheets for you to complete. This form can be obtained online, or by phoning your request to our office. Please fill out the Referral Sheet and fax it to our office as soon as the referral is made. Please call our office to confirm that the fax arrived. The original Referral Sheet can then be hand-carried by the client at the time of the initial appointment, as a double-check that the medical information is received by us. Please call our office if you need additional Referral Sheets.
• Please fax us a Patient Referral Information Sheet even if you only desire a telephone consultation. It greatly assists our office in helping you.
• As part of our Intake Assessment of your patient, we obtain all pertinent information prior to the scheduled examinations as early as possible. Please fax pertinent medical records, including copies of recent lab work and your interpretation of the lab results, as soon as the referral is made. Please also indicate your assessment as to level of anesthetic risk for your patient (low, medium or high, or grade from I to VII), and any adverse drug or anesthetic reactions.
• We must be informed of the patient's temperament and whether a muzzle or sedation, or other means of restraint is required for examination. If, in our estimation, the pet may bite during the exam, he/she will be muzzled or otherwise restrained, without hesitation. We will not examine vicious or aggressive animals.
• After faxing the Patient Referral Information Sheet, please call our office to provide further details about the case if it is complex or if additional information must be given. Indicate if you need to speak directly with Dr. McCalla, after providing this additional information to our staff.
• Please provide a Referral Guide for Patients and Their Owners brochure to the clients that you are referring to our hospital, or our website address, and business cards if needed. Please call our office if you need additional Referral Guides and/or cards.
• If comprehensive physical examination, lab work and/or radiography is requested by Animal Eye Care to be performed at your hospital following our examination, please fax us the results, your interpretation, and your assessment as to level of anesthetic risk present (low, medium, high, or grade I to VII). Please call us, if further consultation with the ophthalmologist is required at this phase of the medical evaluation. Unless otherwise specified, when lab work and other tests are requested by Animal Eye Care to be performed at your hospital, the client will be informed by our staff that YOU will be interpreting the results for the client, and that you will contact our hospital if you feel that consultation with the ophthalmologist about the results is necessary.
• After examining the patient, we will fax you a detailed report of our findings the same day as the examination, and we will call you if additional information must be conveyed to you. Please contact us, if you have any questions regarding our findings or recommendations for treatment.
